Below you will find New Patient Paperwork, Consent Forms & Medical Records Release Forms. These general office guidelines will ensure an amazing experience. If you are scheduling online, you will receive these forms via FormDr. automated electronic forms via email after completing the Schedulicity Online Process. If you would prefer a paper copy, please download, print and complete the New Patient Paperwork and bring it with you prior to your scheduled time. Please do not submit completed paperwork via email. Eye Department has enabled a secure Patient Health Record portal where patients may access their medical records, send messages to Dr. Annie, download current prescriptions and review previous orders. The Eye Department Patient Health Record portal is only available for established patients who have seen Dr. Annie at Eye Department.
